Overall excellent, but some accessible toilets too small to use.
I visited both sites V&A East, and the separate Store room building, about 15 minute walk/wheel away. It was hard to find the musuem from Stratford Elizabeth line station (which is accessible, but you need them to call ahead for a ramp). Google maps may try to take you an inaccessible way as there is a lift out of service, but you don't need to use it after all as you can enter the V&A East on the level you arrive at instead.
The museum was easy to move around in using a powerchair, and nowhere was too bright or noisy. There was a nice calm, dim naturally lit quiet space with stim toys, chairs, blankets, and headphones to use if needed.
Staff at the lower ground cafe were excellent & proactively helpful, they asked me where I'd like to sit, I indicated a nearby table, I ordered at the counter then when I turned around they had already removed the chair to allow my wheelchair to move straight in, without me even having to ask, which was a lovely touch!
The accessible toilets on that lower ground level next to the cafe however were VERY small, almost too small for my powerchair to get into, and there were bins in the transfer space (see photos) so those were not great, but I didn't check any other ones in that building.
In the Storehouse building however they had a superb new changing places bathroom which is spacious and has all the neccessary equipment in good working order that a changing places bathroom requires - shower, adult changing table, hoist etc, accessible by radar key, but staff ask if you have a radar key or if you'd like them to open it for you.
Staff all very attentive and helpful, ready to explain how to access everything. No bags allowed but free lockers of various sizes available (st your own combination when you lock it) to store your bags during your visit, NO liquids are permitted in the storehouse area, so if you need to take meds you will need to return to the reception area where the lockers and cafe are to do that. If you need help to use the storage lockers staff are happy to assist. There are lifts to all floors and I had no trouble navigating around at all.
